Explanation
NOTE
〜拍子に means the moment or just as — at the instant of one action, something else happens unexpectedly.
立ち上がった拍子に、頭をぶつけた。
The moment I stood up, I bumped my head.
NOTE
The second event is usually accidental and unintended — it just happens to occur at that exact moment.
ころんだ拍子に、めがねが割れた。
Just as I fell, my glasses broke.
〜拍子に is often used for small mishaps that occur by chance.
NOTE
Attach 拍子に to a verb in plain past (〜た) form, marking the instant of that action.

拍子にくしゃみをした拍子に、腰を痛めた。
Kushami o shita hyoushi ni, koshi o itameta.
The moment I sneezed, I hurt my back.
ふり向いた拍子に、コップを落とした。
Furimuita hyoushi ni, koppu o otoshita.
Just as I turned around, I dropped the cup.
笑った拍子に、お茶をこぼした。
Waratta hyoushi ni, ocha o koboshita.
