Explanation
NOTE
〜ずにはすまない means must / cannot avoid doing — circumstances make an action unavoidable.
約束を破ったので、謝らずにはすまない。
Since I broke my promise, I cannot avoid apologizing.
NOTE
It is formal and expresses a social or moral obligation — the speaker feels they have no choice.
借りたお金は返さずにはすまない。
I cannot avoid repaying the money I borrowed.
The obligation comes from the situation, not from a personal wish.
NOTE
Attach ずにはすまない to the verb ない-stem. Note that する becomes せずにはすまない.
STRUCTURE

ずにはすまない失礼なことを言ったので、謝らずにはすまない。
Shitsurei na koto o itta node, ayamarazu ni wa sumanai.
Since I said something rude, I must apologize.
規則を破った以上、罰金を払わずにはすまない。

NOTE
〜ないではすまない has the same meaning — must / cannot avoid doing — but is a little more conversational.
こうなった以上、真実を言わないではすまない。
Now that it has come to this, I cannot avoid telling the truth.
NOTE
Attach ないではすまない to a verb in the ない form, simply replacing the final い of ない.
